    How long is the evaluation?

    This will in part depend on the referral question. Typically, an assessment includes an initial intake appointment to gather important clinical information, 1-3 testing appointments (depending on the age of the client and complexity of the referral question), and a feedback appointment to review the results and recommendations.

    What happens after the evaluation?

    After testing is complete, a comprehensive report will be written. This is usually completed within 1-2 weeks, if not sooner. The report will provide a diagnosis (if appropriate) and pertinent recommendations and referrals.

    I will schedule an appointment to review the results with you and discuss any questions you may have. I will also provide you with a copy of the report. With your permission, I may send a copy of the report to the referring provider or other healthcare provider.

    Do you prescribe medication?

    No, I am not a prescribing provider. Upon request, I can provide a referral to a prescribing provider.

    What is your cancellation policy?

    I require 24 hours notice to cancel an appointment. If canceling within 24 hours of an intake appointment, there is a late fee of $30. If canceling within 24 hours of a testing appointment, there is a late fee of $250. There is also a $50 fee for returned checks.
